  • Notice from the Nurse Parent/Guardians please be aware that permission slips are being sent home for your child to received OTC (over the counter) first aid/interventional medication (triple antibiotic ointment, cough drops, etc. ) when needed. Please complete the form and return to the school as soon as possible. Until this form is signed and returned to the school we will be unable to provide these medications if needed.
